All accepted applicants are required to complete and submit this form along with your seat deposit of $400 to hold your place in the entering class. All seat deposits are non-refundable and will be credited toward your tuition and fees for the 2014–2015 academic year.

For the Summer Entering Class: Your seat deposit is due by March 15, 2014, or by the due date indicated in your letter of acceptance.

For the Fall Entering Class: Your seat deposit is due by May 15, 2014, or by the due date indicated in your letter of acceptance.
